I hereby authorize permission for Next Shot Productions LLC to capture my image, likeness, and/or voice through photo, video, or other form of digital medium. I understand that content involving myself can be utilized indefinitely in a lawful manner, including but not limited to promotional, advertising, or marketing purposes. I understand that Next Shot Productions LLC has the right to edit or alter the captured content as deemed appropriate, and I waive the right to review or approve final versions. I understand and agree that I will not receive any compensation for my involvement in the captured content, unless otherwise specified in a separate agreement. I am aware that I can request Next Shot Productions LLC to alter or remove my captured content from the various mediums it may be distributed.
